To schedule an appointment, please call either office Monday through Friday 9:00AM to 5:00PM. Saturday appointments are available at Acton only September through May 9:00AM to Noon.

If you are a patient please bring:
- Completed health history form with HIPPA agreement. Click here to download and print form,
- Your medical insurance card(s),
- Referral if needed – please contact your PCP prior to your appointment,
- Completed Eyeglass Questionnaire Form. Click here to download and print form,
- Contact your insurance company to check your eligibility,
- Eyeglasses (if available),
- Contact lens information – wear them for your appointment and please bring your eyeglasses
What to expect at the conclusion of your appointment
Most patients will be dilated so please bring sunglasses or arrange for someone to drive you home after your visit. Plastic sunshields will be provided to those that need them.
